About Us
KW4 Primary Care Community is a community network, established to link primary care providers – family physicians, nurse practitioners, physician assistants – within the Kitchener-Waterloo-Wilmot-Wellesley-Woolwich (KW4 Region) area to provide succinct and important information related to updates in health services, referral processes, practice opportunities and professional development.
The KW4 Primary Care Network’s aim is to provide one place for the KW4 primary care community to get access to local and current information to help serve your practice and the community.

Mission
We strive to be a trusted community for primary care providers. Our mission is to provide important information about health services, referral processes, practice opportunities and professional development, with the goal of providing one place for the KW4 primary care community to get access to local and current information that is helpful to your practice and serving the community.

KW4 Region
The Kitchener-Waterloo-Wellesley-Wilmot-Woolwich (KW4) sub-region consists of over 400,000 residents, with the majority residing in the municipalities of Kitchener and Waterloo. Approximately 86% of residents live in a large urban centre within the sub-region.
The KW4 sub-region has over 245 primary care physicians serving the community with 90% of physicians located in Kitchener and Waterloo. Less than 20% of primary care providers work in models of care with interdisciplinary health team support. Most physicians who have access to interdisciplinary health teams are in Kitchener. The KW4 region residents, primary care providers, specialists and local hospitals have a strong referral network aligning with the proposed sub-region.
