Dr. Myles Sergeant, Dr. Erin Gallagher, Michelle Howard, PhD and colleagues from McMaster Department of Family Medicine and Hamilton Family Health Team have created a new toolkit, “Sustainable Healthcare for Serious Illness and End of Life Care: A practical guide for Family Physicians and Primary Care Health Teams”.

 

The topics covered will assist family physicians and primary care teams in overcoming barriers when engaging in Advance Care Planning and in providing patient-centered care throughout a life-limiting illness trajectory.
