 In Palliative Care, a multidisciplinary team works closely with your existing supports and professional services and can bring in additional services as needed.  Your care team supports your values and wishes.

Referrals

You can refer yourself or a loved one to Waterloo Wellington Home and Community Care Support Services and a Care Coordinator will work with you, your family and your health care team to develop a care plan that will meet your care needs.

A Physician or Nurse Practitioner in your circle of care may make a referral through Home and Community Care Support Services for specialized Palliative Care Physician or Palliative Care Nurse Practitioner consultation with the KW Hospice Palliative Care Community Team. Consultations take place at our ambulatory clinic at Hospice of Waterloo Region (100 Solstice Way Waterloo, ON N2K 0G3) or, if medically necessary, at home. Any Physician or Nurse Practitioner in your circle of care can always reach a specialized Palliative Care Physician or Palliative Care Nurse Practitioner by phone for advice.
