The KW4 Ontario Health Team (KW4 OHT) is a collective of over 40 member organizations from primary care, home care, hospitals, community agencies, long-term care, mental health, Indigenous health, municipalities and post-secondary education.
Together with patients and families, the KW4 OHT is working towards co-designing a health and wellness system for almost 500,000 residents. This system will offer seamless, interconnected care and continuity across providers.
The KW4 Ontario Health Team is a collaboration of health care partners from all sectors in Kitchener, Waterloo, Wellesley, Wilmot, Woolwich, including primary care, hospital care, long-term care, community health services, and social service groups, who are working together with a goal of providing coordinated care for patients.
Ontario Health Teams exist to help streamline your health and wellness journey, making it easier for you to get the care you need when you need it.
By working together, we can share knowledge, resources, and our collective voice to influence positive change in healthcare. We can coordinate our resources together and make sure everyone involved has access to what they need.
The Strategic Plan is a road map with multi-year goals for KW4 OHT. It articulates the ‘what’ and ‘why’ of our existence and helps us to identify priorities.
Each year the KW4 OHT creates a business plan to guide our work and support our Strategic Plan.
Annually, KW4 OHT develops a Collaborative Quality Improvement Plan (cQIP). A cQIP is a formal commitment to quality that KW4 OHT makes to our community.
The KW4 OHT is evolving its governance structure to strengthen accountability and governance best practices. The OHT has a committee structure in place with representation from the healthcare sector and our community.
The Governance Table (formerly Steering Committee) acts as the KW4 OHT’s oversight body making recommendations and decisions in relation to the KW4 OHT in accordance with the Collaborative Decision-Making Agreement. The Governance Table has responsibilities in different areas of governance for the KW4 OHT, including planning and priorities, resources and accountability, engagement and reporting,
The Governance Table is comprised of five service providers, two family/client, patient or care partner members of the Community Advisory Committee and three community leaders not associated with a health sector service provider.

KW4 OHT recognizes that traditionally this area was a gathering place for many nations.
