The KW4 Ontario Health Team (KW4 OHT) is a collective of 40+ member organizations from primary care, home care, hospitals, community agencies, long-term care, mental health, Indigenous health, municipalities and post-secondary education.
Together with patients and families, the KW4 OHT is working towards co-designing a health and wellness system for almost 500,000 residents. This system will offer seamless, interconnected care and continuity across providers.

Executive Director Bio & headshot: Ashnoor Rahim, Executive Director KW4 OHT
Ashnoor Rahim has joined the KW4 OHT as the Executive Director. With over 25 years of experience in a variety of health and care settings, including extensive work with seniors and marginalized populations. She is a health system leader committed to improving the overall health of Ontarians by creating coordinated and integrated health delivery models that are focused on improving the health of our population. Ashnoor is committed to using evidence based data and clinical best practices to support the overall health and wellbeing of the KW4 communities.
We have included our brand guidelines and a package of our logos to be used by members, partners and community to accurately represent KW4 Ontario Health Team. The Brand Guidelines include our organization's colours, typography, correct logo usage, and best practices to ensure a cohesive brand image.
