The KW4 OHT is committed to developing and supporting the primary care community.
The KW4 OHT works closely with the KW4 Primary Care Network as we forge a path forward for connected and convenient care.
In 2025-2026 we are collaboratively tackling the challenge of access and attachment to primary care providers, guided by the Primary Care Action Plan.
