The KW4 Ontario Health Team (KW4 OHT) is a collaborative of over 40 member organizations uniting primary care, hospitals, home care, community agencies, mental health services, Indigenous health, long-term care, municipalities, and post-secondary education institutions.
Together with patients and families, our mission is to co-design a seamless, interconnected health and wellness system for almost 500,000 residents of Kitchener, Waterloo, Wellesley, Wilmot, and Woolwich. Our team is committed to delivering the right services at the right time and place to meet the needs of our diverse community.
